Follow the attached directions to have department names changed on Bank of America Floors 4 through 37.

Certificate of Insurance Requirements
Prior to inception of any type of operations within the Bank of America Fifth Avenue Plaza, all vendors must provide a current Certificate of Insurance that is in compliance with the specifications and criteria as outlined in the Hines Risk Management Certificate of Insurance Requirements form (attached). Please call Lise Brown at 206-621-9172 with any questions or concerns.

Janitorial Desk Cleaning Request
Use this form to request that the Janitorial Staff clean your credenza or desk-top. Clear off the surface of the area you would like cleaned. Print one form for each piece of furniture and leave it in the center of your desk or credenza where is is clearly visible. For Bank of America Floors, please limit to five (5) desks per night.

Use this form to request duplicate keys and lock changes.

Lobby Directory Change Form
This form may be used by Tenants for any changes, additions or deletions to the interactive main lobby directory. Completed forms can be faxed to 206-621-1251, sent via email to judy_stark@hines.com or brought to the Management Office, Suite 3838.

Overtime Air Conditioning Request
Whenever overtime air conditioning is required at an additional cost please fill out this form and fax to 206-839-8443. Your request will go directly to Engineering. If you do not receive the air conditioning please call Security at 206-839-8437.

Parking Application
Use this form to request parking cards and/or make changes to parking card information. Call 206-624-0864 for further information in regarding to the Bank of America Fifth Avenue Plaza Parking Garage.

Property Removal Authorization
An approved and signed Removal Authorization Form will be required by the Security Officer on duty at all egress points in order to remove equipment from the building.

Umbrella Checkout
Bank of America Fifth Avenue Plaza umbrellas are available for loan to building tenants at the Security Console in the main lobby. On those unexpected rainy days, please feel free to check out an umbrella.
